Form E-536R, Schedule of County Sales and Use Taxes for Claims for Refund (April 2019 - June 2026)

Form E-536R, is to be used in connection with Sales and Use Tax refund claims in order to identify the counties in which county and/or transit tax was paid. This form is used only if the organization made purchases and paid county and/or transit tax in more than one county. This form corresponds to April 2019 - June 2026.
